$100,000 for a Medical Outreach Social Worker who identified resources and community partnerships for chronically homeless persons needing health care. Due to increase in access from those partnerships, Urban Ministry Center has transitioned to four general outreach workers who are able to utilize the work done by the Medical Outreach Social Worker.
